Step 2: Prepare required documents Please note that the list of documents below is not exhaustive. Other documents may be requested if necessary. For the complete list of required supporting documents, please refer to the claim form.
Medical Expenses Claims
• Fully completed claim form
• Original medical and/or hospital bills for injury claims
• Medical certification / doctor’s memo for food poisoning and infectious disease claims
• Final hospital bills for hospitalization claims
• We will request for the Medical Certification form to be completed by the attending doctor if we need more information.
Weekly Benefits Claims
• Original medical certificates
• Medical certification / Doctor’s memo stating nature of injury
• Latest pay slip on letterhead or duly certified by employer
• Other evidence of remuneration which must be duly certified
Death / Permanent Disablement Claims
• Fully completed claim form
• Medical certification and/or doctor’s report certifying the nature of the injury and the latest status of the condition
• For death claims please provide police report, death certificate and coroner’s report
• Letter of Administration / Probate document as necessary for death claims
